The Kingston Pathfinder commissions a range of inpatient and outpatient support for all age groups via Your Healthcare, which became a social enterprise on 1 August 2010.

The service includes support for

  • people with long-term conditions
  • adults with learning disabilities
  • care assessments
  • intermediate care and rehabilitation
  • therapies including occupational therapy and physiotherapy
  • specialist nursing
  • health visiting
  • foot care
  • continence service
  • school nursing
  • speech and language therapy
  • sexual health services

The service also works closely with Kingston Council’s social services department.

You will usually be referred by your GP or other practitioner to access these services but may be able to self-refer in some circumstances.
