The Kingston Care Record is a confidential electronic record of your health and social care information. It is stored on a secure computer system.

What is a Kingston Care Record?

The Kingston Care Record is a confidential electronic record of your health and social care information stored on a secure computer system.

Having your health and social care information in one safe place means your doctor, nurse or social worker will be able to offer you the best care and support quickly and safely.

How will a Kingston Care Record benefit me?

Having your health and social care information in one safe place will benefit you in several ways:

  • You will receive the best possible care, support and treatment more quickly;
  • You won’t be asked the same questions over and over again or have to keep repeating your medical or social care history;
  • Doctors, nurses and social workers will be able to work better together and make the best decisions with you through access to the right information when they need it;
  • You won’t need to have unnecessary appointments or tests;
  • It will reduce the chances of errors being made and so will be safer for you.

 

Who will be able to see my record?

The Kingston Care Record is confidential and secure. Only doctors, nurses and social workers involved in your care and support will be able to see your record when you say they can, or in an emergency.

Your record is only used at the time you are receiving care and support and only after you have given your permission.

Staff not involved in your care and support won’t be able to see your record.

If you don’t want your record to be seen at any time, all you have to do is say so. The choice is yours.

 

 

What information will my record hold?

Your Kingston Care Record holds a summary of your health and social care information from your GP, Community health services, Mental health services, Kingston Hospital and Kingston Council’s Adult Social Care Services. The record will include information about:

  • Your allergies
  • Your medication
  • Any bad reactions you’ve had to medications
  • Any health conditions you may have
  • Doctors, nurses, social workers and care agencies involved in your care
  • Recent medical tests you have had

What if I don’t want anyone to see or share my information?

  1. You can decline at the point of your care, simply tell whoever asks you that you do not want your information shared. The system is designed so that it has to ask for consent and record your response. You therefore have control as to who sees your information.
  2. If you do not want to have your information shared at all: you can opt-out by completing a short form and giving it to your GP surgery. You must provide proof of your identity and place of residence. Once you have opted out your record is completely deleted from the Kingston Care Record system.

 

Please note that opting out altogether may mean that, in an emergency, clinicians may not have up to date information to provide you with the best possible treatment.

If, at a later date, you wish to opt-in, then you can complete an opt-in form and give it to your GP surgery.

If, however, you are happy for your information to be shared on Kingston Care Record, then you do not need to do anything.

Please note: all patients who have opted out of the Summary Care Record or similar will AUTOMATICALLY be opted out of the Kingston Care Record and their data shall not be shared. If these patients wish to be included in sharing their information via the Kingston Care Record they will have to “opt in” by filling in the OPT IN FORM and being coded as such on their record (please see the Opt-In Form).
